Overview:

This hybrid role combines in-person home visits with remote coaching—no nights, weekends, or on-call duties.
Key Responsibilities:
- Conduct home visits to assess safety, monitor health changes, and provide clinical coaching (including medication reviews).
- Deliver ongoing coaching via our digital app, offering resource referrals and best practice guidance.
- Build trusting relationships with caregivers to reduce burnout and encourage positive changes.
- Maintain accurate, timely documentation in compliance with regulations.
- Assist with additional projects and duties as needed.
Requirements:
Education:
- Active Registered Nurse (RN) License in the state of employment.
Experiences:
- 2+ years of recent direct care experience with elders/adults with disabilities.
- Background in case management, care planning, and assessments (eldercare, disabilities, or complex medical needs preferred).
Other:
- Must be able to travel within the assigned area (valid driver’s license and auto insurance required).
- Proficient in MS Office, case management systems, and web-based applications.
- Bilingual skills are a plus (may impact compensation).
Compensation and Benefits:
- Salary: $90,000–$100,000 per year
