Position Overview:

Deliver high-quality, skilled nursing care to residents in a long-term care facility in Queens, NY. Collaborate with interdisciplinary teams to ensure patient well-being, adhere to care plans, and maintain compliance with state and federal healthcare regulations.

Key Responsibilities:

  • Conduct patient assessments and implement individualized care plans.
  • Administer medications and treatments per physician's orders.
  • Monitor and document patient progress in EHR systems.
  • Collaborate with healthcare teams to optimize resident outcomes.
  • Ensure compliance with facility policies and regulatory standards.

Requirements

Qualifications:

  • Education: Valid Registered Nurse (RN) degree.
  • Licensure: Active New York State RN license.
  • Experience: Minimum 1 year of long-term care experience.
  • Technical Skills: Proficiency in patient care documentation and safe medication administration.
  • Soft Skills: Strong communication, empathy, and adaptability in fast-paced environments.
  • Additional Requirements: Immunity to Hepatitis B (proof required).
  • Candidates must have proven experience and working knowledge in the following areas:
    • EMR proficiency
    • Conducting thorough clinical assessments
    • Recognizing and responding to acute changes in condition and completion of SBAR
    • Developing and updating baseline and comprehensive care plans
    • Performing resident admission assessments and documentation
    • Handling of controlled substances and documentation
    • Coordination of care and supervising LPNs and CNAs
    • Fall prevention, Elopement prevention, and response
    • Conducting regular nursing safety rounds
    • Nursing clinical documentation, task management, and follow-up
    • Elder abuse prevention and reporting
    • GG completion
    • PRI and MDS
    • Strong understanding of current best practices in long-term care settings
