Accountabilities:

  • Assess, identify, and secure services tailored to participants’ unique needs while monitoring progress.
  • Support the Project Manager in developing and executing new programs and initiatives.
  • Respond promptly to participant requests and facilitate access to adaptations, medical equipment, transportation, and other resources.
  • Ensure compliance with all documentation, assessments, service plans, and reporting requirements within established timeframes.
  • Maintain detailed, person-centered records of visits and contacts using required systems.
  • Consistently demonstrate organizational values in all professional interactions and duties.

Requirements

  • Bachelor’s degree required; fields such as sociology, social welfare, psychology, gerontology, or criminal justice preferred.
  • Minimum three years of case management experience, preferably within the PA Office of Long Term Living.
  • Strong communication, customer focus, integrity, and problem-solving skills.
  • Proficiency with relevant computer systems and technology.
  • Physical ability to perform duties including reaching, stooping, walking, standing, and lifting up to 20 pounds.
  • Valid driver’s license and reliable transportation to travel throughout Allegheny County and surrounding areas.
